High-Yield NRE Notes
Foregut, midgut, hindgut derivatives, gut rotation, and all major GIT congenital defects — with Meckel’s rule of 2s, Hirschsprung disease, and 10 free NRE-style MCQs.
Why GIT Embryology Matters in NRE
GIT embryology questions in NRE Step-1 always start with a neonate or infant presenting with vomiting, abdominal distension, failure to pass meconium, or a visible abdominal mass. Your job is to identify which developmental step failed based on the clinical picture — not to memorise facts in isolation.
The three most tested areas are the derivatives of the foregut, midgut, and hindgut, the rotation of the midgut and what happens when it goes wrong, and the specific congenital conditions — Meckel’s diverticulum, Hirschsprung disease, duodenal atresia, and omphalocele versus gastroschisis. These topics appear in almost every NRE sitting.
Foregut, Midgut, Hindgut — What Each Becomes
The primitive gut tube is divided into three regions based on blood supply. This is the most important framework for all GIT embryology questions — blood supply determines which region a structure belongs to, and which developmental errors affect it.
| Region | Blood Supply | Derivatives | Key Defects |
|---|---|---|---|
| Foregut | Coeliac artery | Oesophagus, stomach, duodenum (proximal to ampulla of Vater), liver, gallbladder, bile ducts, pancreas, spleen | TOF, duodenal atresia, annular pancreas, biliary atresia NRE |
| Midgut | Superior mesenteric artery (SMA) | Duodenum (distal to ampulla), jejunum, ileum, caecum, appendix, ascending colon, proximal ⅔ of transverse colon | Malrotation, volvulus, Meckel’s diverticulum, omphalocele, gastroschisis NRE |
| Hindgut | Inferior mesenteric artery (IMA) | Distal ⅓ of transverse colon, descending colon, sigmoid colon, rectum, upper anal canal, bladder, urethra | Hirschsprung disease, imperforate anus, bladder exstrophy NRE |
Gut Rotation — How It Works and What Goes Wrong
Midgut rotation is one of the most tested GIT embryology topics in NRE. The midgut herniates out of the abdominal cavity at week 6, rotates 270° anticlockwise around the superior mesenteric artery, and returns to the abdomen by week 10. Any error in this process produces a specific clinical presentation.
Major GIT Congenital Defects — NRE Pattern
Each defect below has a characteristic clinical presentation. NRE questions give you the neonate’s symptoms and ask for the diagnosis or embryological basis. Learn the key distinguishing features for each.
High-Yield Key Points — NRE Checklist
🔸 GIT Embryology — Must Know for NRE
- Foregut = coeliac artery. Midgut = SMA. Hindgut = IMA. Blood supply defines gut region
- Ampulla of Vater divides foregut from midgut — structures above = foregut, below = midgut
- Midgut rotates 270° anticlockwise around SMA — caecum ends in right iliac fossa
- Malrotation → bilious vomiting in neonate = surgical emergency. Ladd’s procedure is the treatment
- Meckel’s diverticulum — Rule of 2s: 2 inches, 2 feet from ileocaecal valve, 2% population, 2 years, 2 tissue types
- Meckel’s = persistence of vitello-intestinal duct. Presents with painless rectal bleeding in a child
- Meckel’s scan = Technetium-99m pertechnetate — detects ectopic gastric mucosa
- Hirschsprung = failure of neural crest cells to migrate to rectosigmoid → aganglionic segment
- Hirschsprung = failure to pass meconium in 48 hours + abdominal distension + explosive release on rectal exam
- Hirschsprung associated with Down syndrome. Diagnosed by rectal biopsy — no ganglion cells
- Duodenal atresia = failure of recanalisation → double bubble sign on AXR → associated with Down syndrome
- Omphalocele = at umbilicus, sac present, liver can herniate, chromosomal associations common
- Gastroschisis = beside umbilicus (right side), no sac, intestines only, not chromosomally linked
- Annular pancreas = ventral bud fails to rotate → ring around duodenum → double bubble sign
- Imperforate anus = part of VACTERL association. Always check for other defects
Test Yourself — 10 NRE-Style Questions
These questions follow the NRE clinical vignette format. Read the scenario carefully — the clinical details always point directly to the diagnosis.
